As frequent readers of this Forum know, there is often discussion on what physical shape you need to be in, and what you should or shouldn't tell the army about your health.
Going forward, you will need to produce a medical document signed by your family physician and including a letter written on his or her letter head.
Below is the official statement by the IDF regarding medical examinations prior to leaving for Israel:
Get a health certificate from your family doctor in your country of origin prior to arriving in Israel, listing your health problems (or confirming that you are healthy). Mahal2000 will publish the official form on the webpage "Volunteering Step-by-Step" as soon as received from the IDF. In the meantime, the health certificate must be signed by your family doctor on his stationery. An official form is available on the IDF's AKA website, however it is not completely clear yet that this is indeed the required form. If you want to avoid any doubt, get both health certificates from your family doctor. - May 18, 2007
